You do not need to print all 130 pages for each client. Read the relevant guide, choose a brief or comprehensive intake, and add the agreements and supporting forms that apply. Use the print index to select complete form ranges, keeping each agreement's explanation and signature pages together. The therapist guides can stay in your reference copy.

For example, an adult packet might begin with an adult intake, emergency contact, communication preferences, customized HIPAA notice, therapy consent, billing form, and welcome sheet. The setup guide also suggests starting packets for teens and children.

FILE USE AND PROFESSIONAL REVIEW

Digital download only; no physical item will be mailed. Download the files from your Etsy purchases. A compatible PDF application is needed to fill and save responses; phone and browser previews vary. Test a saved sample before client use. Typed signature fields are not authenticated digital signatures.

Word edits do not automatically update the supplied PDFs. After editing, check your layout and export a new PDF if needed. Interactive fields must be added separately if you want the newly exported PDF to be fillable.
